Prospective Families

Welcome to Falcon Ridge Middle School - home of the Falcons!  We are excited to have your family and your
student join our school.  In order to register for school it is important for us to learn as much as we can about your
child’s previous educational experience.  Use the New Student Checklist form to be sure you have all the required
information.

• A copy of the student’s Birth Certificate.  If you have lost or misplaced your student’s birth certificate, contact the Dakota County Office of Vital Statistics at 651-438-4313.  If your child was born out of state, contact a similar office in the state.
• For Special Education Students, we require a copy of the most current IEP and Evaluation Report.  Our special education staff will need to review this paperwork to determine appropriate placement.
• Proof of Residency - To verify your residence, we require official documentation of your name and household address.  Common examples of this documentation may include: purchase or rental agreement, driver's license, gas or electric bill.
• Proof of Guardianship - If you are not the parent/s listed on the birth certificate but are a legal guardian, we willneed a copy of legal documentation or a notarized Substitute Caretaker form.

Immunization requirements must be met per district policies and state laws. Please contact our school nurse, Ms. Sikkink at 952-431-8760 for questions. Families should ask for a printed report of immunizations from your medical provider.  Submit this documentation to the school nurse for entry. You can mail or fax it to the health office. (Fax 952-431-8770). It is the responsibility of the parent/guardian to ensure we receive these records and to notify the student’s previous school of your intent to enroll at Falcon Ridge Middle School.

The tour, program overview, and question-answer time take approximately one hour.

 

Thank you for your interest in Falcon Ridge.  For students and families considering Falcon Ridge for grades 6, 7, or 8, we have monthly prospective student/family tours to help you become familiar with Falcon Ridge Middle School. These tour events are intended for families considering moving to Apple Valley/Eagan or open enrollment from schools that do not feed into Falcon Ridge Middle School by attendance boundaries.

We schedule these tours for regular school days, so students and parents can tour and experience the school with students in their normal classroom and class-to-class schedules.

Families from our feeder elementary schools: Glacier Hills, Westview, Echo Park, Thomas Lake, Highland Elementary and Cedar Park are invited to our Parent Orientation event in May.
